Profiles and configuration

An explicit --profile overrides the saved selection. Without a selection, the default name is default.

Removing the selected profile keeps its name selected, so the next account command asks for login instead of switching accounts silently. ELEVENLABS_CONFIG_DIR changes the state directory. Use a disposable directory for tests.
